Best AP Automation for Healthcare 2026

Evaluate healthcare AP tools for the actual vendor data, ERP and clinical-system interfaces, payment controls and required audit exports.

VERIFIED 2026-06-17

Bottom line

Evaluate the required ERP, pharmacy, payment and audit workflow with each vendor. Size alone does not select the platform; verify any 340B functionality and integration scope in a live demonstration.

Is it right for you?

  • Ask every vendor to show you a live 340B purchase order split, not a slide deck about it.
  • Pull your ERP vendor's certified integration list before shortlisting any AP tool.
  • Request a sample CMS audit trail export and have your compliance officer review it before signing.
  • Get references from two organizations with comparable DEA license counts and drug purchasing volume, not just 'healthcare clients'.
  • Confirm whether the vendor stores PHI, and if so, get a signed BAA before any demo that involves real data.
  • Test the PO matching workflow against your actual medical supply catalog, including partial deliveries and backorders, before contract execution.

A healthcare AP evaluation needs to distinguish ordinary supplier bills from pharmacy purchasing and clinical-system records. Define which invoice fields, purchase-order and receiving records, drug identifiers and program records must be exchanged, and which system owns each check. Demonstrate those fields and exceptions with representative supply invoices. A generic AP category does not establish that every product lacks the required data or that the AP module alone validates 340B eligibility.

A healthcare implementation may involve cost centers, formularies, purchasing systems and clinical systems with different responsibilities. Ask each AP vendor to identify the exact Epic, Oracle Health, Meditech or ERP connection proposed, its version support and the data flowing in each direction. Do not assume every vendor lacks a native route, or that middleware is always required.

Ask the compliance team which purchasing records, approval history and credential documents the organization must retain for its applicable rules and contracts. Test retrieval from the proposed AP setup. Set retrieval deadlines from the applicable request and records policy; a universal two-week assumption is unsupported.

HRSA requires covered entities to have mechanisms to prevent a discounted 340B purchase and a Medicaid drug rebate for the same drug. Its Medicaid Exclusion File covers fee-for-service; Medicaid managed care is outside that file. Map the relevant purchasing, dispensing and billing records with the pharmacy and compliance teams. Demonstrate any proposed split-billing connection and supporting record exports; the rule does not establish that every organization must buy one named third-party administrator or that every AP vendor has the same limitation [HRSA Duplicate Discount Prohibition, checked October 12, 2026].

Tool-by-tool breakdown for healthcare organizations

Evaluate Tipalti for supplier onboarding, multi-entity payments and the required tax-document modules. Its AP pricing depends on the selected product and quote; the old $149 figure is not a verified current healthcare package. Have the vendor demonstrate required NDC fields, ERP data flow and audit exports, without assuming every healthcare field or approval hierarchy is supported.

For AvidXchange, obtain the exact connector name, version and maintained scope for the EHR or ERP in use. Demonstrate receiving, partial-delivery matching and cost-center routing on sample invoices, then request a written implementation and volume quote. A hospital bed count does not establish integration superiority or a standard $500–2,000 package.

Stampli documents healthcare invoice capture, coding and approval workflows. Test the required accounting connector and line-item data with representative pharmacy and supply invoices. Request the actual subscription and implementation quote; this review does not establish a universal automation rate, a $500–1,500 healthcare price band or complete controlled-substance workflow support.

BILL can be evaluated for the specific clinic or health-system workflow. Its current AP list prices are $49 Essentials, $65 Team and $89 Corporate per user per month, with Enterprise custom pricing. Automated matching and ERP features vary by tier and accounting software; automatic PO-and-receipt matching is not included in every plan. Demonstrate the required vendor fields, receiving records and exceptions before selecting a tier; invoice count alone does not establish fit [BILL current pricing, checked October 12, 2026].

Concur Invoice needs a proposal covering the actual healthcare vendor data, connector and approval workflow. Do not price Invoice using a blanket $8–10-per-user estimate or assume the SAP brand removes all middleware. SAP publishes starting per-report rates for its Expense plans, which are a separate service from an Invoice implementation.

Ramp and Brex provide bill workflows as well as company cards. Ramp Standard includes bill pay; advanced ERP and Procurement requirements have separate plan or add-on scope. Brex documents supplier bills and accounting integrations in its Essentials setup. Test healthcare-specific fields, payment routing and audit exports for the proposed configuration; neither a small-clinic label nor a general card-platform category proves capability or rules out either service [current Ramp and Brex primary documentation, checked October 12, 2026].

Expensify supports vendor bill collection, document capture, approval and payment, so it should not be ruled out on the premise that it only handles employee expenses. Its Classic help describes workspace approvals and accounting export, with payment method and regional restrictions. That does not establish the healthcare-specific credential fields, EHR connector or PHI service scope needed by a particular organization; test those separately [Expensify Receive and Pay Bills, checked October 12, 2026].

Evaluate Navan for the actual travel and expense workflow. Distinguish BILL Spend and Expense card and budget functions from the separate BILL AP service rather than assuming a card product supplies the entire payable workflow. Ask each proposal to identify the invoice, receiving, approval and payment steps included, along with any separate product or integration needed.

Compliance and regulatory requirements you cannot skip

Have compliance staff define the applicable approval, retention and supporting-document requirements for the organization and record category. Demonstrate invoice approvals, timestamps, responsible users and exports in every shortlisted product. A generic audit-log feature does not certify that any vendor satisfies all CMS or state health requirements.

For controlled-substance purchasing, have pharmacy and compliance staff identify the applicable ordering and recordkeeping process, including Form 222 or CSOS where required. Determine which order and receiving records must be reconciled with the supplier invoice and where that reconciliation belongs. Demonstrate the proposed data transfer and exception handling; the existence of a purchasing rule does not prove that every AP system must store the same DEA or lot-number fields, or that a missing native connector automatically creates a violation.

Identify the state-specific licensing and purchasing requirements that actually apply to the organization and vendor types. Map required credential fields and conditional checks to the proposed onboarding process. Have Tipalti, AvidXchange and BILL demonstrate the same permitted data and exceptions in the quoted configuration; this review has not established a universal ranking or a blanket absence of conditional fields.

Determine whether the invoice data includes identifiable protected health information and whether the provider will act as a business associate for that service. HHS describes the relevant safeguards and agreement requirements; ordinary payment processing is not automatically the same relationship. Confirm the permitted data and service coverage before transmitting PHI. A vendor promise or a BAA alone does not establish compliant use; review the actual safeguards and permitted data [HHS business-associate guidance, checked October 12, 2026].

Integration requirements for healthcare AP

Identify the actual ERP and release, such as Infor, PeopleSoft, Workday or Meditech, before comparing connectors. Ask AvidXchange and Tipalti for the maintained integration scope, field mapping, supported versions and responsibility for updates. Test the same invoices and reconciliations. A certified label or a general cloud-versus-legacy category does not establish which connector will fit this implementation or that every custom connection will fail.

The relationship between AP and the materials management or supply chain system is where most healthcare AP implementations get into trouble. The purchase requisition originates in materials management (often Lawson Supply Chain Management, Infor SCM, or a standalone MMIS like Peoplesoft Supply Chain). The PO is generated there. The receiving record is created there. By the time the vendor invoice arrives in the AP system, the upstream data needs to be present to check a bill against both the order and receipt. If the AP tool cannot pull the open PO and receiving data from the MMIS in near real time, staff are manually keying receipt information into the AP system before they can process the invoice. This defeats a large portion of the automation value. Confirm specifically how the AP vendor handles MMIS integration, not just ERP integration. They are often different systems.

For organizations using Epic, determine whether the proposed finance workflow needs item identifiers from clinical or supply systems and who owns that mapping. Confirm the fields and permitted data with finance, pharmacy and clinical informatics staff. Require Stampli and BILL to demonstrate the actual fields, exported accounting entries and resulting balances in the selected tier. Run representative complex invoices through the demonstration rather than assuming a particular brand handles every clinical-data relationship.

List the actual operating, capital, grant or other restricted accounts and the rules for selecting a funding account. Have Tipalti, BILL, Ramp and Brex demonstrate the same invoice, entity and payment-routing scenarios. Confirm available accounts, permissions and reconciliation in the quoted plan; neither an enterprise label nor a two-to-three-account example guarantees the required routing.

What to watch out for when evaluating vendors

Use representative exception invoices in the demonstration rather than accepting only a vendor-selected happy path. Include a quantity mismatch, a partly received PO and a credit memo, then record which steps require staff intervention. Record the actual exception rate from the sample before estimating staff time.

Healthcare-specific claims are worth interrogating carefully. Vendors have become adept at adding a healthcare page to their website with stock photos of hospital corridors and language about 'supporting healthcare compliance.' That does not mean the product does anything different for healthcare than for any other industry. Ask specifically: does the system have native fields for DEA registration numbers in the vendor master? Can it enforce conditional vendor credentialing requirements by vendor type? Does the approval workflow support the DRG-aligned cost center structure that most hospital finance teams use? If the answers are yes, ask to see those fields in a live environment. If the answers are vague, the healthcare page on their website is marketing.

Obtain a quote tied to actual invoice volume, entities, integrations, implementation and required modules. Review the term and renewal clause and calculate the proposed total cost. Calculate renewal charges from the signed agreement; an assumed 15–20% increase is not a quote.

Build the implementation schedule around connector work, field mapping, migration, permissions and reconciliation tests. Identify the IT and finance owners, acceptance criteria and fallback process for each milestone. Ask the vendor to put those dependencies in the statement of work. A standard 60-to-90-day estimate does not establish this project schedule, and this review has no basis for a universal four-to-nine-month healthcare timeline.

Recommendations by organization size

For an independent practice, FQHC or community health center, evaluate BILL against the required approval, accounting and data-handling workflow. Team and Corporate list at $65 and $89 per user per month, but integration, matching and BAA eligibility need actual scope confirmed. A 340B participant should separately demonstrate the pharmacy and split-billing process; neither a sub-200-person team nor fewer than 500 invoices guarantees fit.

For a regional or multi-hospital system using Infor, Meditech or PeopleSoft, include AvidXchange if the vendor can demonstrate the required maintained connector. Compare matching exceptions, cost-center routing and audit exports with the other proposals. Use the actual quote and project plan, not an unverified $800–2,500 price or four-to-six-month deployment guarantee.

For physician groups and ambulatory practices on Sage Intacct, QuickBooks Enterprise or Microsoft Dynamics, include Stampli in the connector demonstration. Check coding suggestions, expense workflows, payment execution and export reconciliation in the quoted scope. This review has not established a standard $500–1,500 price or automatic simplification of every month-end close.

For a large academic center or multi-state health system, evaluate Tipalti onboarding and multi-entity payment modules against actual payees, required fields and approval routes. Compare the full proposal with other vendors. Fifteen thousand invoices or eight entities does not by itself prove a saving, a complete compliance configuration or universal superiority.

Frequently asked questions

Is BILL HIPAA-compliant for healthcare AP? Check the exact service and agreement. Current BILL General Terms section 6.7 says it may enter a BAA arrangement to certain users who request it, and ePHI must not be sent until a BAA exists. The American Express Vendor Pay help page states a PHI restriction, so do not apply one service statement across every BILL offering. Have the privacy officer confirm the executed coverage and controls; a $49 subscription alone does not authorize PHI use [BILL primary terms, 2026-10-12].

How long does it take to connect an AP platform to an EHR like Epic or Cerner? Ask for the named connector, supported release, direction of data flow and owner of maintenance. Run trial transfers and reconcile ledger totals before approving production use. No source here verifies an out-of-the-box Stampli Epic connector or a fixed four-to-eight-week result for every deployment.

What is 3-way PO matching and why does it matter for hospital procurement? Three-way matching compares the purchase order, receiving record and supplier invoice to identify quantity or price differences. Test partial deliveries, returns and approval exceptions using the purchasing records for the proposed setup. Have the proposed Stampli or Workday setup show its connection, comparison logic and exception assignments; a product name alone does not guarantee every exception is sent to the right manager.

Which platform handles AP automation best for a mid-size hospital system with 200 to 500 beds? A hospital with the stated bed capacity still needs a workflow comparison. Compare the exact purchasing, receiving, clinical-data and financial-system requirements using representative bills, with the PHI permissions explicitly defined. Use the quoted project milestones and subscription charges rather than a presumed Stampli package or guaranteed audit compliance.

What CMS audit trail requirements apply to healthcare AP software? The applicable record category and provider rules need to be identified first. Do not treat a medical-record retention provision as a universal rule for every payable or assume a vendor audit module certifies compliance. Ask compliance staff to specify approval evidence, supporting files, the applicable retention schedule and a retrieval exercise for the proposed system.

What does it cost to automate AP in a healthcare setting? Request separate numbers for the selected modules, invoice and payment usage, connector work, migration and support. The earlier Tipalti149 and Stampli500–1500 estimates do not establish what this organization will pay under a new agreement. Apply the actual quote to expected usage and contractual renewal terms.

What to do next

Most AP and expense tools offer a free trial or demo. We recommend testing 2–3 options with your actual accounting software before committing to an annual contract.
